The Governor appoints members to the SHCC and SACs. Each appointment is for a four-year term unless a person is appointed to serve out the term of someone who has left before the term is expired. Persons may serve a maximum of eight years. This may be two consecutive terms or two partial terms plus full term. Terms begin July 1 and end June 30.

Ms. Jo-Ann Ahuna
is a resident of Hana, Maui. She has served on the Maui County Cultural Resource Commission, Mayor’s Hana Advisory Board for Hana, Hana Community Association Board, American Red Cross, and Ameri Corp. She is currently a Parent Co-Chair Volunteer for the Community Children Council for Hana. She also serves on the Tri-Isle Subarea Health Planning Council.


Mr. Andy Ancheta
is the Oahu County Operations Manager and oversees the Communication Center for American Medical Response. He holds a Master of Public Health degree and Bachelor of Arts (Zoology) degree from the University of Hawaii at Manoa.


Ms. Gae Bergquist-Trommald
is the Executive Vice President for Communications Pacific, where she oversees the marketing side of that company. Ms. Bergquist-Trommald previously served as Senior Vice President for Outrigger Hotels and Resorts for Marketing. She is actively involved in the community, including serving as Commissioner and Chair of the State Foundation on Culture and the Arts, Director (and former Chair) of the Salvation Army and the Washington Place Foundation. Ms. Bergquist-Trommald has been active in the campaign for the Hawaii Theater Center and in the Leader Luncheon of the YMCA of Oahu. She is a member (and former Director) of the Rotary Club of Honolulu. She also serves on the Windward Oahu Subarea Health Planning Council.


Dr. Clayton D.K. Chong
is an Associate Professor of Medicine in the Department of Native Hawaiian Health at the University of Hawaii, John A. Burns School of Medicine, where he received a Doctor of Medicine degree. He also maintains a private practice, specializing in internal medicine and oncology. Dr. Chong’s community and professional activities include Papa Olakahi, Imi Hale and Trustee of Queen’s Medical Center.


Ms. Lili Bryan-Conant
is Chair of the Statewide Health Coordinating Council. She retired from Hawaii Medical Service Association as the Manager of Provider Relations and Contracting – Neighbor Islands. She holds Bachelor’s degrees from the University of California at Santa Barbara and the University of Hawaii at Manoa. She is an active community volunteer, serving on various organizations on Kauai. Ms. Bryan-Conant also served on the Kauai Subarea Health Planning Council.


Ms.
Susan Forbes is President of the Hawaii Health Information Corporation. She graduated from Stanford University with a Bachelor’s degree. She went on to obtain her Master’s and Doctorate degrees in Public Health from the University of Hawaii at Manoa. Her primary interest is developing Hawaii’s healthcare information infrastructure in support of evidence-based health planning, including both delivery system development and health status improvement. She is a member of various professional and community organizations.


Ms. Terri Fujii
is the Managing Partner of Ernst & Young’s Honolulu office and a Partner in its Assurance and Advisory Business Services practice. Her background includes managing and working with clients in the healthcare, not-for-profit, airline, insurance, retail and government industries. She manages the audits for the majority of Hawaii’s leading hospital providers, many not-for-profit organizations including Hawaii’s leading private university, Hawaii’s newest publicly held company, and many other companies and organizations in a range of industries. Terri has been an active and contributing member of the Statewide Health Coordinating Council. In addition to being a CPA, she is a Certified Healthcare Financial Professional through the Health Care Financial Management Association.


Mr. David Heywood
is Executive Director, Hawaii Region, United Healthcare. He also served as Vice President at Hawaii Pacific Health, Chief Financial Officer/Chief Operating Officer at Queen’s Health Plans, and in various financial and management positions at Henry Ford Health Systems. He received his Bachelor of Arts degree in Political Science from the University of Michigan and Master’s in Business Administration from Wayne State University. Community services include Treasurer and Program Services Chair – March of Dimes and Treasurer - Kai Makana (ocean awareness non-profit). He is also past president and current member of the Hawaii Chapter of the Healthcare Financial Management Association.


Mr. Wayne S. Higaki
serves as Senior Vice President and Administrator of North Hawaii Community Hospital. Prior to returning to Hawaii in the late 1990’s, Wayne was a Department Manager at Hoag Memorial Hospital Presbyterian and also Department Manager at Fresno Community Hospital and Medical Center. He has a degree in radiology technology. Wayne is the President of the Board of Directors for the Hamakua Health Center, Vice Chair of the County of Hawaii Workforce Investment Board and a member of the North Hawaii Rotary Club. He also serves on the Hawaii County Subarea Health Planning Council.


Ms. Joanne H. Kealoha
is the Social Services Coordinator for the ILWU, Local 142. She holds a Bachelor’s degree from the University of Hawaii at Manoa and a Master’s degree in Social Work from UCLA. She volunteers with the Oahu Workforce Investment Board and sits on the board for Hawaii Meals on Wheels.


Dr. S. Peter Kim
is a retired Professor of Psychiatry at University of Hawaii of Medicine and is in part-time private practice. He is currently a Clinical Professor of Psychiatry and Program Director, Child and Adolescent Forensic Residency Program at the University of Hawaii JABSOM. Dr. Kim graduated from Seoul National University of College of Medicine with a degree in Doctor of Medicine, Ph.D. in Medical Science-Psychiatry from Toho University/New York University Graduate Schools, and Master of Business Administration from the University of Hawaii College of Business Administration. Dr. Kim is board certified and has license to practice medicine in the states of Hawaii, New York, Georgia, and Vermont.


Ms. Marilyn M. Kunishi
(Ex-Officio) is the Chief of Nursing Services with the Veterans Administration in Honolulu. After graduating with a Bachelor’s degree in Nursing from the University of Hawaii - Manoa, Marilyn earned a Master’s degree in Nursing from the University of California, San Francisco. She is a member of the American Organization of Nurse Executives and of the American Business Women's Association, Imua Chapter.


Mr. Keith Lee
is the Vice President of Legal Services/Regional Counsel for Kaiser Foundation Health Plan, Inc. He was the Corporate Secretary and General Counsel for The Queen Emma Foundation. Mr. Lee received a Bachelor of Arts degree, Magna Cum Laude, in Political Science and American Studies from the University of Michigan. He also received Juris Doctor and Master of Business Administration degree from the University of Chicago. Mr. Lee currently serves on the Honolulu Subarea Health Planning Council. He also serves on the Board of the Native Hawaiian Legal Corporation.


Mr. Creighton Liu
is Business Relationship Manager, HMSA. He holds a Bachelor’s of Business Administration and a Master’s of Business Administration from the University of Hawaii. He was a key participant with SHPDA in the first redesign steps of the Hawaii Health Performance Plan (H2P2). He is on the Board of Directors of the Academy of the Pacific. He also served on the West Oahu Subarea Health Planning Council for eight years.


Mr. Mark Moses
served as a Hawaii State Representative from 1996-2006. He also served for nearly 25 years in the U.S. Marine Corps. He received a Bachelor of Science degree in Physics from University of Texas, Austin. He is now the Director of the Hawaii Office of Veterans Services. He has been active in a wide variety of community activities and organizations, most notably in the geographical areas of Makakilo/Kapolei/Honokai Hale, Barbers Point and Ewa. He is a founder and former Vice Chair of the Makakilo/Kapolei/

Honokai Hale Neighborhood Board, Policy Committee Member of the Oahu Metropolitan Planning Organization, and Community Representative of the City and County of Honolulu Planning Advisory Group for the Ewa and Central Development Plans. He also serves on the West Oahu Subarea Health Planning Council.


Ms. Bonnie Lee Pang has a Bachelor of Arts degree in International Relations from the University of the Redlands and a Masters in Business Administration from the University of Phoenix. Ms. Pang has served on the Windward Subarea Health Planning Council. She is currently the Benefit Consulting Manager at Atlas Insurance Agency, Inc. Previous employments include Market Business Leader/Principal for Mercer Health & Benefits – Hawaii, Accounts Manager for Kaiser Foundation Health Plan, Inc., and Presentation and Communications Coordinator for the Hawaii Medical Service Association. Ms. Pang is a member of the Society of Human Resource Managers, Hawaii Society of Corporate Planners, Hawaii Venture Capital Association, Young Business Council, Past President for the Hawaii Association of Health Underwriters, and Board Member of the Punahou Alumni Association.


Dr. Virginia Pressler
is Senior Vice President, Strategic Business Development with Hawaii Pacific Health. She held the position of Deputy Director of Health Resources at the Department of Health and previously was in private practice as a general surgeon, associate professor of surgery, and held various positions as physician executive in the healthcare community. She holds a MD degree from the John A. Burns School of Medicine, MS in Physiology, Masters in Business Administration from the University of Hawaii, and Bachelor of Arts in Social Psychology from Cornell University. She is a member of several professional organizations and is active in various community agencies.


Mr. Frederick Rohlfing
is a retired lawyer and a former member of the Hawaii State Legislature (Minority Floor Leader and Minority Leader). He served in Viet Nam as a Naval Officer. He was past president of Kula Community Association, member of the Kula CA Community on commercial bike tours, and member of the Maui County Salary Commission. He has received letters of commendation as a Naval Officer, and received numerous awards, including the Maui Affordable Housing Alliance Meritorious Service award, The Maui News award for “who made a difference in 2001”, and competitive swimming medals.


Mr. Kenneth Shimonishi
has nearly two decades of working experience in the health care arena. This includes serving as accountant, director of accounting services, and financial planning supervisor at the Kauai Medical Group and Hawaii Pacific Health. He currently is Accounting Manager for Princeville Operating Company, LLC. Born and raised on Kauai, Mr. Shimonishi offers a good perspective of the issues and concerns relating to health care services of the island. He also serves on the Kauai Subarea Health Planning Council.


Ms. Sarajean A. Tokunaga
is a very active member in the community. Currently, she is President of Hui No Ke Ola Pono and Vice President of Papa Lokahi. She is a Board member of Maui Memorial Medical Center Foundation and Board of Director member (and past president) of the Japanese Cultural Society of Maui. She also served on the Tri-Isle Subarea Health Planning Council for eight years.


Ms. Patricia Uyehara-Wong
is Vice President - Administration & Corporate Secretary of Hawaiian Electric Industries, Inc. and previously served as Vice President, Corporate Excellence with Hawaiian Electric Co., Inc. In her current role, she oversees the medical and related benefits of HEI’s employees and retirees. She graduated cum laude with a Bachelor of Arts from Yale University and earned her Juris Doctor degree from the University of Pennsylvania Law School. She is a member of the Hawaii State Bar Association and serves various community service organizations, including Goodwill Industries of Hawaii, the Hawaii Employers Council, and the Honolulu Symphony.

Kauai County Subarea Health Planning Council


Mr. Alan R. Kimura
is employed by the Princeville Corporation. He attended Kauai Community College. He is active with the community coaching soccer and basketball and is a member of the North Shore Community Planning Association for Princeville. Alan was recognized as Employee of the Quarter at Princeville Corporation.


Ms. Betty Moore
is a retired real estate agent. She is very active in the community. She has been serving on the Wilcox Foundation Board since 1992. Ms. Moore is a member of the Wilcox Auxiliary, President of the Sun Village Community Association, active with the Kauai Police Department in their First Responders for Sudden Deaths, and member of the Office of Elderly Affairs for approximately 20 years. She also served in previous years on the YWCA Board.


Mr. Zachary Octavio
is a Paramedic and Operations Manager for American Medical Response on Kauai. He is a graduate of Kapiolani Community College where he received an Associate degree. Zachary is active in the community. He provides paramedic medical support to the Kauai Pop Warner Association and is a car seat technician for the Kauai Keiki Injury Prevention Coalition.

Mr. William “Speedy” Bailey
is the Hawaii General Manager at American Medical Response and holds a Bachelor’s degree in Biology and Psychology from Occidental College. He is a very active volunteer and serves on various organizations relating to his professional and community interests. He was a member of the Statewide Health Coordinating Council for eight years.


Ms. Jennifer Diesman
is the Director of Government Relations for HMSA. She has a Master’s degree in Public Administration from the University of Wisconsin-Madison and also attended Oxford University. Prior to joining HMSA, she was the communications director with Rosehill and Associates, a senior program manager with Hawaii’s Alternative Fuels Task Force, and served in the Executive Office of the State of Wisconsin. She serves on the Board of Directors for Helping Hands Hawaii, Hawaii Bicycling League, and the Hawaii Association of Health Plans. She is currently a Co-Chair of the Honolulu Subarea Health Planning Council.


Ms. Mary Dixon
is a retired social worker from the Executive Office on Aging, Department of Health. She has a Bachelor’s degree in Psychology and a Master’s degree in Social Work. Mary has spent a significant portion of her professional career working with older adults and actively participates in various community and professional organizations.


Ms. Joan Dobbs
is Assistant Specialist at the University of Hawaii at Manoa. She is a nutritionist, researcher, and educator who specializes in food composition and health education. She has held numerous health-related positions, extensively publishes and co-authors a health options column in the Star-Bulletin. She is currently a Co-Chair of the Honolulu Subarea Health Planning Council.


Mr. Keith Alan Lee

Mr. Jonagustine Lim
is a Senior Information Technology (IT) Consultant for the American Chamber of Commerce in Shanghai, China. He is also CEO and President of LimPacific, Inc., Honolulu, Hawaii. He has over seven years of IT consulting and project management experience for government, tourism, and non-profit industries. He also volunteers in several community-related activities such as Alliance for Drama Education, Ohia Productions, Pacific & Asian Affairs Council, and the Filipino Chamber of Commerce in Hawaii.


Mr. Lloyd Lim
is the Health Branch Administrator for the Hawaii Insurance Division, Department of Commerce and Consumer Affairs. He supervises health insurance regulation, including rate regulation, financial surveillance, complaints, investigations, licensing, legislative matters, and external reviews of health plan coverage denials. He implements the loss mitigation grant program. He also has experience in loss and debt financing.


Ms. Sarah Nordwall
was the District Coordinator and is now working part time as a Fashion Consultant for Carlisle Designer Clothes in Hawaii. Ms. Nordwall began Sarah of Hawaii and was a buyer for Liberty House Department Stores. Her community activities include serving as a Board Member for Achievement Rewards for College Scientist, the Rotary Club of Honolulu, Hawaii Theatre, and Central Union Church.


Mr. Emmet White
is President and Chief Executive Officer for Arcadia Retirement Residence. Licensed as a Nursing Home Administrator, he is also a qualified Assisted Living Administrator. In 1968 he received a B.A. from Lafayette College and in 1971 graduated from the College of William and Mary with a J.D. He is an active member in the community serving on committees and boards involved with senior living and long-term care programs. A retired Army Reserve colonel, he serves as an Ombudsman for the National Committee for Employer Support of the Guard and Reserve.

Ms. Anne Atkinson
currently works in Representative Kimberly Pine’s Office as a Legislative Aide researching and drafting monthly newsletters and constituent mailings. She has worked as a speechwriter for the U.S. Embassy-London and has served as Country Director for the USAID Program in Macedonia.


Mr. David Chang
is Assistant Vice President –Business Integration and Intelligence Services at the Hawaii Medical Services Association. Over the past year, his working experience has focused around discovering and implementing high technology solutions. Previously, he served as an Executive Vice President in charge of technology and special projects, as well as running the Retail Division at Central Pacific Bank. He also served at Bank of Hawaii for a number of years. He has a Bachelor’s of Science degree in Electrical Engineering from the University of Hawaii Manoa. He is active in various community organizations, including serving on the board of a church in the Kalihi-Palama area.


Mr. Mark Moses

Mr. Frederick Shaw
is the Senior Analyst of Medical Economics for Kaiser Permanente. He held positions of financial analyst, database analyst, information center manager and financial manager in various health care organizations. He has Master’s of Business Administration and Bachelor of Science degree from California State University. He has held key positions in many community organizations, especially in the Waianae area.


Ms. Leslie Tanoue
is a Manager with Alu Like, Inc., Elderly Services Program. In this capacity she is responsible for supervising managers on five islands. In addition, she coordinates the implementation of the Stanford University’s Chronic Disease and Self Management Program at Alu Like, Inc. sites. Ms. Tanoue has a Master’s degree in Public Health and has a background in gerontology.



Windward Oahu Subarea Health Planning Council


Ms. Gae Bergquist-Trommald

Mr. Greig E. Gaspar
is active in the community. His involvement in community service includes the Native Hawaiian Education Council-Oahu Island, the Board of Directors of Hina Mauka, the Committee for the Miss Hawaii USA, and Miss Hawaii Teen USA Pageant. Mr. Gaspar holds a Masters degree in Secondary Education from the University of Hawaii.


Mr. Christopher J. Lutz
is the Health Care Administrator for the Windward Area Clinics of Kaiser Permanente. He has a Master’s in Business Administration degree from Chaminade University and a Bachelor’s in Industrial Engineering from UC-Berkeley. He served as Project Management Engineer in the Hawaii Region and Management Engineer with Kaiser Permanente in Oakland, California. Since 2003, he has been the President of the Tantalus Community Association, and is a regular participant in the quarterly Tantalus Work Days to improve the Tantalus environment and its impact on the Ala Wai watershed.


Mr. Christopher Sibley
is Program Manager at HMSA. He previously served as Senior Consultant, Primitive Logic Inc. (technology consulting firm), Vice President of Originations at Deutsche Bank Mortgage Capital, Real Estate Project Manager, University of San Francisco, and Senior Credit Officer at Hibernia National Bank. He holds a Master’s in Business Administration from the University of San Francisco and Bachelor of Science in Finance from Louisiana State University. He is active in community work including HMSA community events, Na Ala Hele, American Cancer Society and Make-A-Wish Foundation.



Tri-Isle Subarea Health Planning Council


Ms. Jo-Ann Ahuna

Mr. Glenn Izawa
is a Social Worker with the Hawaii Department of Health-Adult Mental Health Division on Molokai. He has a Master's degree in Social Work from the University of Hawaii, Manoa. Glenn is a member of the National Association of Social Workers, board member of the Molokai Health Foundation, Board Chairperson of the Molokai Community Services Council and of the Selective Service Board.


Mr. Curt S. Morimoto
is the Paramedic Operations Manager for American Medical Response ambulance service in Maui County. He was born and raised on the Big Island and graduated from St. Joseph High School in Hilo. He holds a Bachelor of Science degree from Seattle University. His paramedic background goes back to 1976. He served five years as a commissioner of the Maui County Department of Fire and Public Safety, ending in March 2004. Currently, he serves as a Liquor Commissioner for the Maui County Department of Liquor Control. He also is a child passenger safety technician and instructor.


Ms. Valerie L. Simonsen
is a licensed naturopathic physician, shaman, and ceremonialist based in Maui. She received her Doctorate from the National College of Naturopathic Medicine, Portland, Oregon, and a Bachelor of Science degree in Biology from Montana State University. She travels the world offering complementary healthcare and distinguished ceremonies of empowerment aiding people to attain wholeness with passion, beauty, and truth.


Ms. Elaine Slavinsky
is the Maui program coordinator for the Alzheimer’s Association. She has a Masters degree in Public Health, a Masters degree in Education from the University of Hawaii, a Bachelor of Arts degree from the University of Massachusetts, and an Associate degree in Nursing from Massachusetts Bay Community College. She previously served as the Clinical Director for Hospice Maui, the Director of Nursing for Maui Memorial Medical Center, and in various positions in several hospitals and health centers in Hawaii. She is a member of the Hawaii Nurses Association, Hawaii Public Health Association, American Association of Nurse Executives – HI Chapter, Hospice and Palliative Nurses Association, and the American Association of Hypnotherapists.


Ms. Kathleen Suzuki
has over 30 years of experience in the healthcare arena. For over two decades, she served as a medical technologist at Kaiser Permanente. She also served at the Maui Memorial Medical Center. She is a member of the American Society for Clinical Laboratory Science, Hawaii Society for Clinical Laboratory Science, and American Society for Clinical Pathology.


Dr. William Thomas
is an internist and currently serves as Medical Director – Clinical and Internal Affairs, Chief of Staff, Director of Long Term Care at the Molokai General Hospital. He is an Alumni of Kamehameha Schools, University of Hawaii, and John A. Burns School of Medicine. He has received numerous honors and awards including “Resident of the Year” from the University of Hawaii Internal Medicine Residency Program, 2002, and “Most Compassionate Resident of the Year/Mother Marianne Award” from St. Francis Medical Center.

Mr. Thomas O. Brown
is the Director of the County of Hawaii Mass Transit Agency, where he has worked for 27 years. He worked together with the Hawaii County Subarea Health Planning Council for several years in the development of their transportation project. He received his Bachelor’s degree in Public Administration from the University of Hawaii, Hilo.


Mr. Wayne Higaki
is Senior Vice President and Administrator of North Hawaii Community Hospital. Mr. Duane Ilstrup
is a retired biostatistician and was an Associate Professor of Biostatistics at the Mayo Clinic in Rochester. He earned his Bachelor of Arts degree in Mathematics and Physics at Augsburg College and Master’s in Science Biometry degree from the University of Minnesota. He has sat on numerous boards, made presentations at national and international meetings, performed design and analysis of randomized clinical trials on research interests, provided biostatistical consulting in various clinical areas and has an extensive bibliography.


Mr. Karlson Pung
is Paramedic and Operations Manager for American Medical Response in Hilo. He attended the University of Hawaii, Colorado Audio Institute and Kapiolani Community College. Karlson is also a self-employed musician. He is active in his community.


Mr. Christopher Schlueter is an attorney with the Law Offices of Steven Strauss in Hilo. He has interests in business law, real estate/land use, immigration law and litigation. He represents clients in a variety of civil litigation matters. Previously, he served as counsel to attorneys regarding proper redemption law procedure. He is a graduate of Xavier University in Cincinnati, and Gonzaga University School of Law, in Spokane.


Ms. Gail Walker
serves as Clinic Manager at Kaiser Permanente in Hilo. Her prior experience includes serving as Executive Director of the Hamakua Health Center, Director of Nursing at Kapiolani Women’s and Children’s, and Nursing Supervisor, and staff nurse. In addition, she has received numerous professional and community awards including the Robert Wood Johnson Community Leadership award, Weinburg Foundation Aim for Excellence award, and Outstanding Citizen Hawaii County. She also works with East Hawaii Coalition for Homeless and the Hawaii Island Adult Day Care.
